OFFICE VISITS
New Patient Consult Appointment — $385.00
New patient consult appointments can be face-to-face or via phone or Zoom for long-distance patients—the cost is the same.
Follow Up Appointment for Patients — $175.00
A follow up appointment is needed to review all lab, saliva test and thermal image results. The follow up appointment is NOT included in the cost of any lab work or testing.
Review of Outside Imaging Reports (Non-Patients) — $225.00
If you have had outside breast thermal imaging or other imaging results you would like to discuss, Lynn offers a 30-minute appointment to review the results and make recommendations regarding your individual breast health.
TESTING
Hormonal, Adrenal Function and Other Testing
There are a variety of testing types and levels available, and costs vary based on a variety of factors. Testing costs and approach will be discussed at your initial visit..
Self-billing insurance (other than Medicare) may be possible. Please talk to your insurance company directly.
If you are interested in self-billing your insurance, please be aware of the following:
NOT all plans allow self-billing, and Medicare does NOT permit patients to self-bill.
It is the responsibility of the patient to consult their insurance plan for instructions and appropriate forms for self-billing.
At the time of your visit you will receive an itemized receipt with necessary insurance codes to facilitate self-billing.
It is advised to make copies of all forms prior to submitting insurance claims.
Lynn Chadd is an Out-of-Network Provider/Non-Preferred Provider who is not contracted with any insurance companies.
CANCELLATION
A 48-hour notice is required for all patient appointment cancellations.
We reserve the right to charge in full for your appointment if you:
cancel with less than 48-hour notice
fail to show up for your scheduled appointment time
